Our CareSpan Cancer Management program supports your employees at every stage from preventive care to end-of-life support.
- HealthPartners provides your employees with preventive reminders by phone and mail for early detection of breast, colorectal and cervical cancer.
- A Cancer Care Coordinator supports your newly diagnosed employees by reaching out to their oncologist to confirm the diagnosis and validate how the Coordinator can integrate with the care team to add value.
- After confirming a new diagnosis, the Cancer Care Coordinator provides phone support for your employee. Support includes developing a care plan, expectations for treatment, managing side effects and shared decision-making.
- The Cancer Care Coordinator also helps survivors access follow-up care.
- When necessary, the Cancer Care Coordinator helps your employees utilize and understand advance directives, palliative care and hospice care.
