All HIPAA covered healthcare providers, both individuals and organizations, must obtain an NPI for use in identification and HIPAA standard transactions.

Submitting your NPIs to HealthPartners
Once you have applied for and received your NPIs, please notify HealthPartners of your numbers as soon as possible. Use the tabs below to determine the best method of communicating your NPIs.
Please contact your HealthPartners Service Specialist prior to submitting an NPI only on your claims submissions. Submitting NPIs on claim submissions is not considered notification to HealthPartners of NPI numbers and could result in incorrect or non-payment.

What is a National Provider Identifier (NPI)?
The NPI is a unique numeric 10-digit identifier that will be assigned by the Centers for Medicare & Medicaid Services (CMS) to all covered providers of health care who transmit health information either electronically or on paper.

Type 1 NPIs - are issued to providers or individuals who render health care. Examples include: physicians; dentists; nurse practitioners; chiropractors; pharmacists; and physical therapists. |

Type 2 NPIs - are issued to health care providers and organizations. Examples include: hospitals; home health agencies; clinics; nursing homes; residential treatment centers; laboratories; ambulance companies; group practices; suppliers of durable medical equipment; and pharmacies. |
How is a NPI obtained?
Providers receive their NPI after applying either in paper or electronic form through the National Plan and Provider Enumeration System (NPPES).

Type 1 NPI Information
Submit your Type 1 Practitioner NPIs to HealthPartners
Select a process below that works best for you in reporting your Type 1 NPIs to HealthPartners. A HealthPartners representative will review the information and contact you with any questions.

Provider Data Profiles
Enter your Individual (Type 1) NPIs directly into HealthPartners online systems using the Provider Data Profile application. This application allows you to review and update your practitioner and facility information online at your convenience.

Minnesota Uniform Credentialing Application
Practitioners who require credentialing should include their NPI number on the Minnesota Uniform Credentialing Application and fax it to 952-853-8702.

Minnesota Uniform Demographic Change Form
Enrolled practitioners (those practitioners who do not require credentialing) should include their NPI on the Minnesota Uniform Practitioner Change Form Minnesota Uniform Demographic Change Form and email to Provider Data Support.
It is recommended to use secured, encrypted email service to send private and protected information via email.
Type 2 Information
How to submit your Organizational NPI (Type 2) to HealthPartners
Submitting NPIs on claim submissions is not considered notification to HealthPartners of NPI and could result in incorrect or non-payment. HealthPartners supplies an easy way to submit your NPIs online using the form below.
Providers can submit Type 2 NPIs quickly and directly to HealthPartners using an easy online form. This form includes fly-over descriptions of the fields needed to report your NPI information.
A HealthPartners representative will review the information and contact you with any questions. If you submitted more than one NPI, we will notify you when our systems have been updated and you can begin NPI submission on your claims.
It is recommended to use secured, encrypted email service to send private and protected information via email.

Unique Minnesota Provider Identifier (UMPI)
National Provider Identifiers (NPIs) are the federally-required standard unique health identifiers that must be submitted on all transactions and paper submissions as of May 23, 2008. Some providers do not qualify for an NPI and fall under the atypical classification. The Minnesota Department of Human Services (DHS) has assigned and mailed atypical practitioners a 10-digit Unique Minnesota Provider Identifier (UMPI). DHS assigned UMPIs are required to be submitted on electronic claim and paper submissions for atypical providers.
If you are an atypical provider and you have not received an UMPI assigned by DHS, please visit the Department of Human Services web site for additional information and instructions.
