Description:Participating in the pharmacy experience survey provides valuable feedback that will help identify areas to focus improvements and help improve future service for you or others at your local HealthPartners and Park Nicollet pharmacies.
Selected patients that have been to a pharmacy in the last six months are mailed a one page survey to complete and return in a prepaid envelope. Those that have not completed the survey by mail may be contacted by phone to complete this short survey.
