Given the growing prevalence of diabetes and the increasing proportion of adults with type 2 diabetes who are not meeting their glycemic targets (28–32), new approaches to initiating and intensifying insulin therapy in type 2 diabetes are needed. In this article, we describe a safe and simple insulin titration algorithm that addresses many of the obstacles that clinicians and patients encounter when adding mealtime insulin to a basal-only insulin regimen.
