Here’s some additional information about getting care with your plan:

• The UnityPoint Health ACO network is made up of the UnityPoint Health care system and select provider partners. The plan does not offer out-of-network benefits.
  
• If you have the Network plan, your network may be split into tiers depending on where you live. The plan does not offer out-of-network benefits.
• Emergency services anywhere in the U.S. are covered as in-network care (but follow-up services after an emergency may not be).
• For routine chiropractic care, you can see any chiropractic provider. You don’t have to see an in-network provider for these services.
• Only certain weight loss surgery providers (PDF) are in network.
