Accountable Care Organization
Park Nicollet and HealthPartners Medical Group participates as an Accountable Care Organization (ACO) in the ACO Realizing Equity, Access and Community Health (REACH) Model. An ACO is a group of doctors, hospitals and other health care providers who agree to work together to keep you healthy. All members of Park Nicollet Health Services ACO, LLC, d/b/a HealthPartners ACO agree to work together to give their patients better quality care. We transitioned from the Direct Contracting Model to the ACO REACH Model in 2023.
