Hospital-level care at home for acute illnesses

Sometimes, illnesses require hospitalization in order to meet treatment needs. But in some cases, it’s possible to meet these needs outside of a traditional hospital setting.

At HealthPartners and Park Nicollet, we know that many people, if given the choice, would prefer to get treatment at home rather than in a hospital bed. The HealthPartners Hospital@Home program is a way to do exactly that.

Through the program, participants receive daily visits from specially trained clinicians for medication, treatment and education, along with regular video visits with a doctor – often the same one you would see if you were at the hospital. It’s not only a comfortable and convenient option, but it can also lead to better health outcomes.

HealthPartners Hospital@Home is a referral-based program. If you or a loved one require hospitalization for an acute illness or similar health event, ask your care team about your eligibility.

Who is eligible for the HealthPartners Hospital@Home program?

To be eligible for the HealthPartners Hospital@Home program, patients must meet certain criteria, which can include things like:

  • Being at least 18 years old
  • Currently receiving emergency or inpatient care at Regions or Methodist Hospital
  • Experiencing an acute health event (such as a urinary tract infection, asthma flare-up or chronic obstructive pulmonary disease flare-up) that requires hospitalization
  • Living within a 20-mile radius of Regions or Methodist Hospital
  • Normally able to take care of daily needs alone or with the help of current caregivers
  • Managing pain without narcotic pain medication

What do hospital-at-home services include?

If you’re eligible for the program, you’ll go home from the hospital, and we’ll send you any medications you need from our inpatient pharmacy. Then, you’ll be visited by a member of your care team each day that you’re hospitalized at home. Your team will provide many of the same services you would receive in the hospital, including:

  • Administering intravenous (IV) and oral medication
  • Bedside lab testing
  • Electrocardiogram (ECG) monitoring
  • Oxygen titration
  • Some X-rays and ultrasound scans

Each visit will typically last between 30 to 90 minutes. Your clinician will review your care plan, give you a physical exam, check your vitals, administer medication and collect samples as needed. You’ll also have time to discuss your care and any questions you may have.

In addition to in-person visits, you’ll have regular check-ins with other members of your care team over video and by phone. Once you no longer need hospital-level care, your team will ensure that any necessary follow-up care is planned and transitioned to the appropriate care providers.
