Who qualifies for Medicare medication therapy management?
You’re eligible for our Medicare MTM program if you have a HealthPartners Medicare plan with prescription drug coverage. These plans include:
Our Medicare Advantage plans in Minnesota - Most of
our Medicare Advantage plans in Wisconsin HealthPartners® Minnesota Senior Health Options (MSHO) plan- Group plans with prescription drug coverage
If you have one of these plans, you may automatically be enrolled in our MTM program. If you’re not automatically enrolled, you can still sign up for MTM by calling
You’ll also be automatically enrolled in our Medicare MTM program if we determine, or your previous plan determined, that you could be at risk for misuse of certain frequently abused prescription drugs.
- You take eight or more medicines for chronic conditions
- Your expected medicine costs are greater than $1,623 in a single year
- You have three or more of these conditions:
- Alzheimer’s disease
- Bone disease, including osteoporosis, osteoarthritis or rheumatoid arthritis
- Congestive heart failure (CHF)
- Diabetes
- Dyslipidemia
- End-stage renal disease (ESRD)
- Human immunodeficiency virus (HIV) or acquired immunodeficiency syndrome (AIDS)
- Hypertension
- Mental health issues, including depression, schizophrenia, bipolar disorder, or other chronic or disabling mental health conditions
- Respiratory disease, including asthma, chronic obstructive pulmonary disease (COPD) or other chronic lung disorders
You’ll receive an email or letter in the mail confirming your enrollment and providing guidance on next steps. You can schedule an appointment or opt out of the program.
How much does Medicare medication therapy management cost?
If your HealthPartners Medicare plan includes prescription drug coverage, the MTM program is available at no cost to you.
Does Medicare Part D cover medication therapy management?
Medication therapy management isn’t a Medicare Part D benefit. But it’s included at no cost with most HealthPartners Medicare Advantage plans. These plans include Medicare Part D coverage that’s separate from, but works alongside, your medication therapy management benefit.
To enroll in MTM, all you need is a HealthPartners Medicare plan with prescription drug coverage.

The MTM program is not considered a Part D benefit.
From Oct. 1 through March 31, we’re available 8 a.m. to 8 p.m. CT, seven days a week. From April 1 through Sept. 30, we're available 8 a.m. to 8 p.m. CT, Monday - Friday.
