What is medication therapy management?
MTM pharmacists specialize in taking a top-to-bottom look at daily medicine routines. While your doctor focuses on diagnosing and treating health conditions, MTM pharmacists focus just on the medicines and supplements you take – how they’re working, how they’re interacting and how they’re impacting your lifestyle.
MTM pharmacists take the time to get to know your current overall health and the role your medicines play in helping you feel better. They’ll work with both you and your doctor to suggest adjustments that maximize your health and well-being, including potential drug alternatives that may help you save money.
Who qualifies for medication therapy management?
You’re eligible for our MTM program if you have a HealthPartners plan with prescription drug coverage. These plans include:

MTM enrollment depends on what kind of HealthPartners plan you have:

How does medication therapy management work?
MTM involves one-on-one appointments with a pharmacist in person, over the phone, or through your computer or tablet. During appointments, your pharmacist will look at your current prescriptions and ask questions about how they’re working for you and how often you take them.
Your pharmacist will also ask questions about other medicines you might be taking, including over-the-counter drugs and supplements. From there, your pharmacist will see if there are any drug interactions that might cause side effects, plus if there are any combinations that could work better for your health.
Your pharmacist might also see if there are other drug options that may be just as effective but cost less. After appointments, your pharmacist will work with your doctor and care team to adjust your prescriptions if necessary.
