Who qualifies for medication therapy management?
You’re eligible for our MTM program if you have a HealthPartners Medicare plan with prescription drug coverage. These plans include:
Our Medicare Advantage plans in Minnesota ,Most of our Medicare Advantage plans in Wisconsin ,HealthPartners® Minnesota Senior Health Options (MSHO) - Group plans with prescription drug coverage
If you have one of these plans, you may automatically be enrolled in our MTM program. If you’re not automatically enrolled, you can still sign up for MTM.
You’ll also be automatically enrolled in MTM if we determine, or your previous plan determined, that you could be at risk for misuse of certain frequently abused prescription drugs.
- You take eight or more medications for chronic conditions
- You have three or more of these conditions: congestive heart failure (CHF), diabetes, end-stage renal disease (ESRD), depression and chronic obstructive pulmonary disease (COPD)
- Your expected medicine costs are greater than $5,330 in a single year
You’ll receive a letter in the mail confirming your enrollment and providing guidance on next steps. You can schedule an appointment or opt out of the program.
How much does medication therapy management cost?
If you have a HealthPartners Medicare plan with prescription drug coverage, the MTM program is available at no cost to you.

The goal of an MTM program is to ensure your mix of medicines is safe, effective and a match for your lifestyle. This resource aims to help you understand and manage your medicine so you can receive the best care at the lowest total cost.
Does Medicare Part D cover medication therapy management?
Medication therapy management isn’t a Medicare Part D benefit. But it’s included at no cost with most HealthPartners Medicare Advantage plans. These plans include Medicare Part D coverage that’s separate from, but works alongside, your medication therapy management benefit.
To enroll in MTM, all you need is a HealthPartners Medicare plan with prescription drug coverage.

Already enrolled in MTM? It’s easy to find an appointment with a clinical pharmacist near you. (Please confirm with your pharmacist that they’re in your Medicare plan’s network.)
What to expect at your MTM appointment
You’ll have a private one-on-one appointment with a clinical pharmacist. Clinical pharmacists specialize in working with patients to make sure their medicines are safe and effective and match their lifestyle.
