Lipedema basics

Lipedema causes unusual amounts of fat to build up in the lower body. It’s common, but it can be easily mistaken for other conditions like obesity or lymphedema. It mainly affects the legs and butt. It less commonly affects the arms. And it almost never affects the trunk (stomach area).

Lipedema usually begins as a cosmetic concern. But it can progress to cause pain, swelling and bruising. There are four stages of lipedema:

  • Stage 1 – In this stage, it can be difficult to distinguish between lipedema and normal weight gain. Signs of lipedema typically include extra fat in the butt, thighs and calves, pain when pressure is placed on the affected areas, and skin that doesn’t indent when pressed.
  • Stage 2 – Stage 2 lipedema can result in noticeable, lumpy fat deposits under the skin that look like cellulite. It can also result in discoloration and bumpiness.
  • Stage 3 – At this stage, large fat deposits at the knees and excess leg fat are common, as well as noticeable skin folds. Fat accumulation may make walking difficult.
  • Stage 4 – Without proper treatment, lipedema at this stage can turn into lipo-lymphedema, where fat can block lymph nodes and result in fluid buildup. Some common indicators of this stage are whole-body swelling, uneven fat distribution in the legs and lipedema progression to the arms.

Lipedema mostly affects women. It’s not known exactly what causes it. But it’s thought to be influenced by hormones because it tends to start or worsen around puberty, pregnancy and menopause.
