Credentialing and enrollment

If you are not already contracted:

Before credentialing can begin, you must be a contracted provider or have been offered a contract.

Provider Data Profiles is an easy way to manage all your locations and practitioners:

Contracted provider groups can make additions and updates to location and practitioner listings using the online Provider Data Profiles application. This tool offers very specific profile information you can review, add, and update:

  • Locations: addresses, appointment phone numbers, services, hours, and more
  • Practitioner: credentialed information, specialties, personal profiles, affiliated clinics and hospitals and more
  • Make your changes online and save them as a PDF to submit to other parties!

Sign in to manage your profiles. If you don’t have a Provider portal user account or the Provider Data Profiles application, contact your delegate.


When a practitioner joins a HealthPartners contracted clinic, the practitioner or clinic must submit a credentialing application.

It is recommended that an application be submitted at least 90 days prior to employment date. Practitioners may not see HealthPartners members and will not appear in directories or in any Member Service resources until the credentialing process is completed and the application is approved.

It is imperative that HealthPartners be notified when a practitioner leaves a contracted clinic.

It is recommended to use secured, encrypted email service to send private and protected information via email.

 

How long does the credentialing process take?
The process can vary depending on completeness of the application, whether additional information needs to be obtained from the practitioner, and whether the practitioner meets all HealthPartners credentialing criteria.

 

Practitioners will receive written notification once their application has been approved.
