What’s included on your bill from St. Francis Ambulatory Surgery and Endoscopy Center

St. Francis Ambulatory Surgery and Endoscopy Center is a free-standing Ambulatory Surgery Center, also known as an ASC. Your insurance may have a specific coverage benefit for this place of service. Our ASC will bill the facility fee(s) for your services, and the professional fee(s) will be billed by your provider's clinic. Knowing this ahead of time can help you plan for your care expenses.

We’re committed to meeting the medical needs of our community. This includes offering financial assistance for those patients who need care but have difficulty with expenses.

How to apply for financial assistance

Apply for financial assistance online with your HealthPartners account, or download the application in English (PDF), Spanish (PDF), Somali (PDF), Vietnamese (PDF), Hmong (PDF), Russian (PDF) or Karen (PDF).

Complete the application and provide the necessary income verification. This may include copies of your most recent federal tax returns or other verifications of household income. If you’re currently receiving disability income, provide a copy of your most recent award sheet as well as a copy of your most recent tax return. The financial assistance application must be completed in its entirety to process the application.

Application forms must be returned in person, by mail or by fax. Our address and fax number are included on the application for your convenience. When your application and documents arrive, they will be reviewed.

Assistance is based on federal poverty guidelines, size of family, household income and expenses, etc. We expect our patients to pursue all sources of payment for expenses before help can be granted.
